Apport de l’Échographie dans l’Orientation Diagnostique des Ménométrorragies à l’Hôpital Régional de Ngaoundéré

RESUMEObjectifs : Determiner la prevalence des menometrorragies (MMT), ses etiologies a l’echographie et faire une confrontation echo clinique. Methodologie : L’etude etait descriptive transversale concernant toutes les femmes de 15 ans et plus, presentant les MMT, realisant une echographie pelvienne de juin a novembre 2017. Les variables etudiees etaient : l’âge, les indications, l’abondance du saignement, la gestite et les resultats d’echographie. L’echographe etait de marque Logiq 7. L’echographie etait realisee par voie sus pubienne et/ou endovaginale. L'analyse des variables etait realisee avec le logiciel Sphinx plus version 5.1.0.6, Microsoft Excel 2010. Resultats. 90 echographies pelviennes pour MMT etaient retenues, soit 23,3% des echographies pelviennes. L’âge moyen etait de 32,98 ans. 76% avaient deja ete enceintes; 22% avaient pris les contraceptifs, 3% les anticoagulants. Les etiologies retrouvees a l’echographie etaient, myomes interstitiels (24,4%), dystrophies ovariennes poly micro kystiques (16,7%), kystes simples ou cloisonnes (12,2%), myomes sous sereux (10%), myomes sous muqueux (8,9%), kyste dermoide (6,7%), polypes (5,6%), abces tubo ovarien (3,3%), neo de l’endometre, neo du col uterin et hyperplasie de l’endometre (2,2% chacun), Adenomyose (1,1%). 25% d’echographies etaient normales. Les femmes de 15 a 30 ans avaient pour la plupart une echographie normale, celles de 30 a 50 ans les myomes interstitiels et celles plus de 50 ans le neo de l’endometre et du col uterin. Conclusion. Les MMT sont plus frequentes chez les femmes jeunes, l’echographie permet le plus souvent de retrouver des etiologies qui sont soit fonctionnelles ou organiques. ABSTRACTObjectives. To determine the prevalence of menometrorrhagia (MMT), its etiologies in ultrasound and to make an echo-clinical confrontation. Methods. The study was descriptive cross-sectional for all women aged 15 years and over, presenting MMT, performing a pelvic ultrasound from June to November 2017 in ngaoundere regional Hospital. The variables studied were: age, indications, bleeding quantity, gestity and ultrasound results. The ultrasound machine used was GE logiq 7 with convexe probe of 3,5Mhz and endocavity probe. Ultrasound examinations were performed simultaneously by supra pubic and endovaginal route. The variables was analysed with Sphinx plus version 5.1.0.6, Microsoft Excel 2010. Results. 90 pelvic ultrasounds for MMT were retained, representing 23.3% of pelvic ultrasounds. The average age was 32.98 years with extremes of 19 and 52 years, . 76% had already been pregnant; 22% had taken contraceptives, 3% anticoagulants. The etiologies found on the ultrasound were interstitial myomas (24.4%), poly micro-cystic ovarian dystrophies (16.7%), single or partitioned cysts (12.2%), sub-serous myomas (10%), submucosal myomas (8.9%), dermoid cyst (6.7%), polyps (5.6%), ovarian tubo abscess (3.3%), endometrial cancer (2,2%), cervix cancer (2,2), endometrial hyperplasia (2.2%), Adenomyosis (1.1%). 25% of ultrasounds were normal. Most women between the ages of 15 and 30 years had normal ultrasound, those between the ages of 30 and 50 years, interstitial myomas, and those over 50 years, endometrial cancer and cervix cancer. Conclusion. Menometrorrhagia are more common in young women. Ultrasound allows most often to find etiologies that are either functional or organic.
