Observance au Traitement Topique Anti-Glaucomateux : Enquête auprès de Trois Cents Patients

RESUME Objectifs. Evaluer l’observance du traitement des patients atteints de glaucome primitif a angle ouvert au CHU IOTA de Bamako au Mali. Population et Methodes. Il s’agissait d’une etude transversale descriptive realisee sur une periode de 6 mois incluant tous les patients atteints de glaucome primitif a angle ouvert sous traitement medical topique, depuis au moins 6 mois et dont l’âge est superieur ou egal a 40 ans. Resultats. L’âge moyen des patients etait de 58,24±10,28 pour des extremes de 40 et 78 ans. Le sexe masculin a ete le predominant soit 55,7%. L’observance globale etait bonne chez 222 patients soit 73,35%. Les raisons de l’inobservance etaient principalement liees a l’oubli (43,75%) suivi du cout eleve (31,25%). Il n’y a pas une association statistiquement significative entre l’âge, le sexe, le cout du traitement, le schema therapeutique et l’observance au traitement. Cependant le respect des rendez-vous de suivi et le respect des horaires d’instillation etaient significativement lies a l’observance au traitement. Conclusion. Les principales raisons liees a la mauvaise observance sont l’oubli et au cout eleve de la prise en charge. Pour une bonne adhesion a la prise en charge du glaucome primitif a angle ouvert, il s’avere necessaire l’integration et le renforcement des strategies multidimensionnelles prenant en compte l’oubli et le cout du traitement. ABSTRACT Objective. Potentially blinding, primary open-angle glaucoma (POAG) is a chronic progressive optic neuropathy corresponding to loss of retinal ganglion cells. Blindness by POAG can be avoided or at least delayed if the patient is compliant to the treatment. The aim of this study is to assess treatment compliance in patients with primary open-angle glaucoma at the CHU-IOTA in Bamako, Mali. Population and methods. A descriptive cross-sectional study was carried out over a period of 6 months including all patients with primary open-angle glaucoma under topical medical treatment, for at least 6 months and whose age is greater than or equal to equal to 40 years. Results. The mean age of the patients was 58.24 ± 10.28 for extremes of 40 and 78 years. The male was predominant at 55.7%. Overall compliance was good in 222 patients, or 73.35%. The reasons for non-compliance were mainly related to forgetfulness (43.75%) followed by high cost (31.25%). There is not a statistically significant association between age, gender, cost of treatment, regimen, and adherence to treatment. However, adherence to follow-up appointments and adherence to instillation schedules were significantly associated to treatment compliance. Conclusion. The main reasons related to poor compliance are forgetfulness and the high cost of care. For good compliance to the management of primary open-angle glaucoma, it is necessary to integrate and strengthen multidimensional strategies considering forgetfuness and the cost of treatment.
