Ceramic overlay in endodontically treated teeth: a conservative and biomimetic approach

Résumé

Background. Restoring endodontically treated teeth presents challenges related to structural loss, biomechanical changes, and esthetic considerations. Selecting an appropriate indirect restoration supports long-term function and tooth preservation. Case report. A 38-year-old female patient presented with an esthetically compromised maxillary first molar previously restored with a large amalgam filling. After removal of the defective restoration, a composite resin core build-up was performed to reinforce the remaining structure, followed by placement of a monolithic zirconia overlay. The restoration’s marginal integrity, functional stability, and esthetic integration were evaluated according to FDI criteria at the one-year follow-up. Conclusions. The combination of a composite core build-up and zirconia overlay provided a conservative approach that preserved enamel, optimized bonding, and supported functional occlusion. These findings illustrate the potential benefits of biomimetic, minimally invasive strategies for restoring structurally compromised endodontically treated teeth; however, further clinical studies are needed to confirm predictability and long-term outcomes.
